Initial Consultation (for individuals visiting the hospital for the first time)

  • Reception times
    From 8:30 a.m. to 11:00 a.m.
  • Reception Place
    Initial Consultation Desk (window②) on the 1st floor of the East Wing.

Please bring a physician’s referral which is to “Hiroshima City Hiroshima Citizens Hospital” for the initial consultation. Patients who do not have a physician’s referral will be asked to pay an additional 5,500 yen non-referral initial consultation surcharge (or 3,300 yen for a dental consultation). In addition, please note that depending on the department, patients may not be able to see a doctor without a physician’s referral.

Window tickets will be issued from 8:00 a.m.

Note that initial consultation reception for the Ophthalmology Department on Tuesday id from 8:30 a.m. to 10:00 a.m.

Also,please note that consultation days may differ among departments.Ask the Initial Consultation Desk for more information.

Outpatient services are closed on Saturdays,Sundays,holidays,Aug.6,and from Dec.29 to Jan.3.

If you are visiting this hospital for the first time, please take a window ticket during the reception times specified above and wait until you are summonded Then take your ticket to the Initial Consultation Desk (window 2).
  • Before this, please complete the Patient Examination Form found on the document counter, and give this along with your health insurance card or medical care certificate (social welfare, senior care etc.) and physician's referral to the clerk.
  • We will register the information you provided on the form and create a hospital card for you. We will also schedule a same-day appointment and give you a reception ticket. You will be summoned by the number on this ticket instead of your name. Please keep this ticket until you submit it to the accounting desk at the end of your visit.
  • After processing your reception, the clerk at the Initial Consultation Desk will hand you a yellow folder containing your reception ticket and hospital card and will also return your health insurance card separately.

Please be aware that in some cases same-day appointments may not be available.

Also, please note that waiting time may be long because reserved patients have a priority.

Your arrival will be confirmed when you submit the yellow folder to the reception desk of the department which will be examining you.
  • The departmental reception desks are found on each floor and are called "block receptions". Please submit your yellow folder to the block reception clerk to confirm your arrival for examination.
  • Note that your turn may be indicated by an electronic screen found either on the reception desk or in the waiting room. If this is the case, please proceed to the inner waiting area or the examination room when your number appears.

Please notify the clerk when you leave the waiting room and when you return.

If you have a same-day appointment,you will be summoned by the number on your reception ticket instead of your name. Therefore, please keep this reception ticket until you submit it to the accounting desk.

If you are visiting a department which is different from the one you were seen by before, or if you have not visited the hospital for a long period of time, it may be treated for the first visit.

Please follow the directions below.
  • Patients with appointments (reception from 7:30 a.m. to 5:00 p.m.)
    Please swipe your patient card through the automated reception machine on the 1st floor of the East Wing, confirm that the information appearing on the screen is correct, and then press the "Finish" button. The machine will print out a reception ticket with your reception number and appointment details. Please submit this ticket along with your hospital card to the block reception of the department you are visiting.
    Please note that in order to solve the congestion and shorten the waiting time of the patients, the automated reception machine can accept your hospital card only from 90 minutes before the appointment time of consultation or examination whichever is earlier.
  • Patients without appointments (reception from 8:30 a.m. to 11:00 a.m.)
    Please take a window ticket and go to the Initial Consultation Desk (window 2) when your number is summoned if you are visiting a department which is different from the one you were seen by before, or if you have not visited the hospital for a long period of time.
  • Please be aware that in some cases same-day appointments may not be available.

Please go to the General Reception Desk on the 1st floor of the East Wing.
  • The clerk at this desk will give you a hospital card that has been prepared for you beforehand. Please swipe this card through the automated reception machine, obtain a reception ticket, and submit it to the block reception of your designated department.

After Your Examination

The yellow folder will be returned to you in the examination room following your examination.
  • A form titled "Basic Outpatient Schedule" will be added to the folder. Please follow the instructions on this form indicating where you need to go after the examination.

A form titled "Appointment Schedule" will also be given to you if a follow-up appointment has been arranged.

When all examinations and treatments have ended, please proceed to the accounting desk (window 3) and hand your yellow folder to the clerk.
  • Your reception number will be called after your fees have been calculated. Please pay at either the cashier desk (window 4, Hiroshima Shinkin Bank) or at the automated teller machine.

A prescription exchange ticket will be attached to your receipt if you are to receive medication from the hospital pharmacy.

A prescription exchange ticket will be given to you by the accounting desk if you do not have to pay any fees, but have been instructed to receive medication from the hospital pharmacy. In this case, please proceed to the accounting desk when your reception number is summoned.

A confirmation stamp will be placed on your prescription by the accounting desk (window 3) if you are to receive your medication at a pharmacy outside the hospital.

  • Please visit the "Fax Prescription Service Corner" as you may be able to order your prescription via fax.

Obtaining prescription medication from the hospital pharmacy (window 5)
  • When your prescription exchange ticket number appears on the pharmacy screen, proceed to the pharmacy counter (window 5), hand your ticket to the clerk and obtain your medication.

Miscellaneous Items

Please prepare your health insurance card or medical care certificate when you return to the hospital for the first visit of each month. In addition, please notify the appropriate desk as soon as possible if there have been any changes in your insurance status or information.
  • A bill in the name of the insured individual will be sent if you use an insurance card that has expired.
Rescheduling appointments
  • Please call the department you are scheduled to visit directly between 2:00 p.m. and 4:00 p.m. if you need to reschedule your appointment. The dermatology, plastic surgery, otorhinolaryngology/audiology, pediatrics, and pediatric surgery departments do not allow rescheduling if you call on or after the date of the original appointment.
Hospital card
  • The number on your hospital card will also be used for your patient records. This permanent number will be used by all departments. Therefore, please keep this number ready for visits and phone calls to the hospital.
  • If you lost the card, it will be reissued after the procedure at the Initial Consultation Desk on the 1st floor. However, a 100-yen reissue fee will be charged.
